The idea
Heart Care Healthy Living is a community-based medical charity working to support people recovering from heart and lung conditions. Building on our clinical expertise, we’ve developed Keep It Healthy—a medically supervised programme for those at risk of chronic illness, including high blood pressure, diabetes, high cholesterol, arthritis, and obesity.
This initiative offers tailored exercise, lifestyle support, and emotional well-being sessions, including Tai Chi. We aim to expand access to the wider public, starting with a pilot focused on men’s mental health, in partnership with Aldridge Rotary, who are helping us reach this group.
We also hope to revive our previously successful weight management service, which complements Keep It Healthy. In order to deliver these expanded services we will need to purchase an exercise cross trainer. With the right funding, we can empower people to take control of their health before crisis strikes—building a stronger, healthier community.
What we'll deliver
- Deliver an 8-week exercise programme for participants in our gym with support from clinical staff throughout
- Deliver a 12-week weight management & exercise programme for participants in our gym, supported by clinical staff
- Purchase an exercise cross trainer to be used by participants and leaving a legacy item for patient use long term
Why it's a great idea
Keep It Healthy offers more than just exercise—it’s a lifeline for those at risk of chronic illness. Over 8 weeks, patients receive expert-led physical activity, tailored weight management, and emotional support to help them take control of their health. We monitor progress closely, aiming to reduce risks linked to high blood pressure, diabetes, and obesity. But the real impact lies in how it makes people feel: stronger, more confident, less fatigued, and more connected. With seated options and Tai Chi, it’s inclusive and empowering. For those who feel isolated, it’s a chance to belong. This programme doesn’t just change habits—it transforms lives.
Steps to get it done
- Confirm staff availability & levels & hours, and finalise programme materials
- Purchase 1 Exercise Cross Trainer, 3 marketing banners and leaflets
- Recruit participants for the exercise programme with help from Aldridge Rotary
- Recruit participants for the weight management programme through local partners
- Work the Walsall Council and promote the service to various local organisations & groups
- Market to local GP surgeries & health centres
- Agree measures and outcomes to be recorded with Walsall Council
- Launch 8-week tailored exercise sessions twice weekly
- Begin 12-week weight and lifestyle management sessions
- Final Assessments and Feedback - Reassess participants at the end of the programme
- Share Results and Celebrate Success - showcase outcomes and testimonials to show the difference your support made
