The idea
Diabetes is a long-term and progressive condition with potentially devastating consequences for health which is often associated with unhealthy body weight and a leading cause of heart attack, stroke, kidney failure, visual loss, and memory loss. All the indications suggest that the prevalence of diabetes will continue to increase in the years ahead.
Provides one-on-one and group health promotion. This will offer participants individualised programme with an emphasis on overcoming personal barriers to aid health goals and prevention of risk and complications of diabetes.
To encourage physical activity, healthy cooking and stress management skills through local health champions (including GPs, Nurses, Dietitians, Sports, Yoga/Tai-Chi Practitioners ) and Mental Health awareness sessions.

Why it's a great idea
Diabetes is one of the most pressing health and social care challenges facing us today. Diabetes currently affects three million people in the UK and this is expected to rise to 6.2 million by 2035. This increase will place significant pressures on the health and social care system.
The hidden impact of diabetes in social care is that there are over 70,000 people with diabetes incurring local authority social care costs and the direct care costs is estimated as £1.4 billion annually.
Hence, the project is to provide multi-facets approach by Diabetes Action CIC to address the impact and burden of diabetes by providing coordinated approach to meet the needs of individuals at risk and those with Diabetes by providing them with tailored health programme that will improve their health outcomes.
Steps to get it done
